Аннотация

Цель исследования – провести сравнительный анализ изменения когнитивных функций, состояния гемодинамики и уровня седации у больных с врожденными пороками сердца (ВПС) в условиях эндоваскулярных оперативных или диагностических процедур.

Материал и методы. В одноцентровое проспективное рандомизированное исследование были включены 70 пациентов с ВПС, которые подверглись эндоваскулярным лечебно-диагностическим процедурам. Пациенты были разделены на две группы по 35 человек: группа тотальной внутривенной анестезии (ТВВА) и группа ингаляционной анестезии (ИА). Проведен сравнительный анализ гемодинамики, времени индукции и пробуждения пациентов, глубины седации, которая контролировалась по уровню биспектрального индекса (BIS). Исследование когнитивной функции проводилось с помощью Монреальской шкалы оценки когнитивных функций (МоСА-тестирование). На основании полученных результатов осуществлялся сравнительный анализ исходных когнитивных функций и их статуса после операций, сравнительный анализ в группах анестезии.

Результаты. Пациенты обеих групп были сопоставимы по исходным показателям гемодинамики. По мере выполнения оперативного или диагностического вмешательства межгрупповой разницы по этим показателям обнаружено не было (р > 0,05). Пациенты находились в седации по шкале RASS от -4 до -3, показания BIS-монитора в группе ТВВА 58 ± 3,83, а в группе ИА 57 ± 2,40, межгрупповая разница отсутствовала (р > 0,05). В группе ИА перед оперативным вмешательством при МоСА-тестировании пациенты набирали 27 ± 1,38 балла, в группе ТВВА 27 ± 1,62 балла (р = 0,98). На следующий день после операции – 27 ±1,43 и 27 ±1,67 балла соответственно (р=0,96).

Заключение. Применяемые анестетики могут вызывать нарушение когнитивных функций в раннем послеоперационном периоде, что может быть зафиксировано посредством МоСА-тестирования. Севофлуран и пропофол оказывают минимальное воздействие на гемодинамику во время индукции и на основном этапе эндоваскулярной процедуры позволяют провести ее максимально гладко, при необходимом уровне глубины седации, что дает возможность хирургу выполнять манипуляции в стабильных условиях, а анестезиологу – быть уверенным в надежности проводимой анестезии.
