Abstract

Recently, with the accumulation of information, our capabilities in understanding the pathogenesis of diastolic dysfunction (DD) and heart failure with preserved ejection fraction (HFpEF) as a multicomponent process, in the development of which many similarities have been revealed: inflammatory changes leading to coronary and periphery endothelial dysfunction and many differences in phenotypes. At the same time, there is a large divergence in the course of various diseases that lead to HFpEF from coronary heart disease, arterial hypertension, the development of renal failure to atrial fibrillation, increased the left ventricle and the left atrium myopathy with the development of pulmonary hypertension, insufficiency of the right ventricle. In the pathogenesis, there may be not one risk factor or trigger, but many acting together in one direction – pathogenetic polymodality. Recently, the hypotheses of the DD development have been supplemented by new studies, which are also aimed at finding universal methods of treatment.
