Abstract

Surgical management of complex multilevel left ventricular outflow tract obstruction often includes enlargement of the aortic root (Konno aortoventricuioplasty), and it is problem to choose a valve for the aortic valve replacement. Taking the prosthetic valve disadvantages into consideration, the replacement of the aortic valve by a pulmonary autograft with the anterior aortoventriculoplasty seems to be appropriate for those patients. We analyzed early and medium-term results in two groups of patients: first after the Ross-Konno procedure and second after the Konno procedure including the aortic valve replacement with a mechanical prosthesis. Absence of any significant differences between two groups and autograft hemodynamic advantages make the Ross-Konno procedure the procedure of choice in surgical management of this category of patients
