Abstract

Predicament of flow-out from lesser circulation has been demonstrated in 31 mitral stenosis patients with 1,6-3,6 cm2 mitral orifice area with the aid of perfusion pulmonary scintigraphy and / or reopulmonography. The impediment in 48% of cases combined with an increase of systolic pressure up to 33 - 49 mm Hg measured by indirect manometry in pulmonary artery. Turbulent trans-mitral flow in mitral stenosis patients has been shown to be an indipendant mechanism of flow-out impediment from lesser circulation and a predictor of pulmonary arterial hypertension registered with incidence 7,5 times higher on the background of vortex flow than laminar one. Increased pressure in the pulmonary artery caused right ventricle hypertrophy in combination with slowing-down of its myocardium relaxation, the latter evoking classic-type trans-tricuspidal flow changes. 90% of the patients with mitral orifice area
