Peculiarities of cerebralhemodynamics in adult patients with secondary atrialseptal defect

Abstract

39 patients with interatrial septal defect (IASD) and open oval foramen (OVF) were examined: 6 patients (15.4%) - with functioning ineratrial connection and 33 patients (84.6%) after correction of defect. 31 patients (79.5%) were examined retrospectively after 1 - 2 years after endovascular correction of IASD using septal Amplatzer occluders and 2 patients (5.1%) - 5 and 6 years after IASD closure. Surgical efficiency of endovascular IASD correction was 91%. During the study we revealed the dynamics of clinical manifestations after IASD correction, determined the types and estimated the dynamics of headache, studied peculiarities of central and cerebral hemodynamics (arterial and venous). Headache was noted in 82% patients with IASD, migraine was present in 28% of them. 91.7% cases of headache in preoperative period, regardless of its type, persisted in postoperative period. Headache episodes disappeared after IASD closure only in 7.7% cases. In 97% patients with headaches had vascular pathology: venous cerebral outflow disturbance - in 60%, associated arteriovenous pathology - in 30.3%, isolated arterial pathology - in 6.7% patients.
