Abstract

Surgical treatment of patients cardio-vascular profile, especially in operations with cardiopulmonary bypass is associated with disorders of hemostasis, which are accompanied by topical or systemic hemorrhage, local thrombotic complications or disseminated intravascular coagulation. Systemic inflammation is joined condition of postoperative infection, associat- ed with the release of cytokines, adhesion molecules, acute phase proteins and initiates the generation of thrombin and activated coagulation factors, which by means of receptors, protease-activated, with its side mononuclear cells to stimu- late the release of tissue factor portions new and inflammatory mediators. Processes of amplification and extension of inflammation and blood clotting, mediated by tissue factor, reduce the mechanisms of regulation of natural anticoagulant and lead to inhibition of fibrinolysis, intra- and extravascular fibrin deposition in organs, caused their clinical dysfunction. The modern concept of prevention, diagnosis and treatment of disorders of hemostasis in cardiac surgery personalized includes clinical and laboratory diagnosis of the real-time, use of activation markers of hemostasis and control methods of antithrombotic therapy.
