Abstract

Objective – to determine criteria for ultrasound diagnosis of an ocular ischemic syndrome in patients with carotid stenosis.
Material and methods. We prospectively studied the clinical manifestation of ocular ischemia and hemodynamic changes, using Duplex ultrasound in 165 patients with carotid stenosis.
Results. Ultrasound signs of ocular ischemia were identified: flattening of systolic Doppler spectrum waveform of ophthalmic artery (OA), central retinal artery (CRA) and its branches, retrograde flow in the ophthalmic artery, lack of CRA blood flow due to occlusion in 80% of cases, decreased Vs 1.5–2.5 times in acute and chronic ocular blood flow disorders, increased RI 1.5–1.6 times compared with the control group, in all investigated ocular arteries, as well as the absence of significant hemodynamic changes in the arteries in patients with sudden monocular loss of vision (between attacks).
Conclusion. Establishing ultrasound diagnostic criteria in ocular ischemia can contribute to early detection of carotid artery stenosis, and to prevent ocular and cerebral circulatory disorders and complication.
