Abstract

Objective: evaluation of antithrombin activity in patients with coronary heart disease in combination with mitral valve pathology at various stages of surgical intervention.
Material and methods. The study included 25 adult patients who underwent surgery on the mitral valve in combination with myocardial revascularization (coronary artery bypass grafting). Measurement of the test markers was conducted in two stages – before skin incision and after the closure of the surgical wound.
Results. An antithrombin activity before the operation was at 78 (69; 105) % and was not differ between patients with acute and chronic disorders of the mitral valve – 72 (67; 86) % and 86 (72; 112) %, respectively (p = 0.23). The second phase of the study revealed a statistically significant decrease in anti-thrombin activity – 59 (49; 72) % and 48 (44; 60) %, respectively, more significant in patients with chronic mitral valve impairment (16% vs 44%, p = 0,01).
Conclusion. Patients with an impairment of mitral valve in conjunction with coronary artery disease have initially reduced level of antithrombin, which is further reduced in the surgical correction of the heart disease. This effect is more pronounced in patients with long-existing defect of the mitral valve. This category of patients needs in antithrombin activity monitoring and, if necessary, its correction for the prevention of thrombosis and rationalization of heparin thetapy in the early postoperative period.
