Trends of blood component therapy in a multi-disciplinary clinic

Abstract

Objective – analysis of trends hemocomponents therapy in a multidisciplinary clinic.
Material and methods. The data based on the reports retrieved from the Automated Transfusion Information Data System on preparation and use of the donor blood components in the Blood Center of the Sechenov University for the period from 2007 till 2018 have been used. Statistical analysis of the data obtained were conducted using application software package Statistica 6.0 and Microsoft Excel 2013.
Results. Total number of the patients admitted to the surgical units of the Clinical Center of the Sechenov University has been steadily growing for the last 5 years. Performance indicators of the Blood Center for this period principally correlate with the data across Russia – increase of employee blood donation proceeds and it has favorable effect on the infectious safety of blood components (frequency of blood donation exemptions based on specific infectious markers found in the back donors was 5.4 times higher than that of employee blood donors), and the level of apheresis platelet preparation increases. Almost threefold decrease of plasma preparation by automated apheresis method is characteristic for the recent year. These developments are connected with the changes in demands of the Sechenov University clinics for transfusion media – increase of demand for erythrocyte-containing components as well as washed erythrocytes, platelet concentrate and cryoprecipitate is observed. At the same time, use of freshly prepared plasma has been constantly decreasing in the recent years due to limited indications for use thus making its processing into such products as lyophilized plasma and albumin relevant. Amount of the washed erythrocytes has been increasing from year to year but the most significant increase coincides with the three recent years. Steady increase of the clinical hospital demand for cryoprecipitate is also observed.
Conclusion. Summing up our experience of transfusion guide use in multi-disciplinary clinical hospitals of the Sechenov University for the recent years, we can conclude on marked and stable trends of changes in the structure of the patients’ blood component supply resulting in the general change of blood transfusion care strategy. Conventional preparation of the donor blood components is also structurally changed– preparation of plasma for clinical use by the automated apheresis method becomes less relevant. Role of employee blood donation constantly increases for provision of timely and complete plasma quarantine achieved in the process of erythrocyte preparation. Research studies on the new blood-derived products are carried out based on the Blood Center in order to increase efficiency of the care delivered in the clinics of the Sechenov University.
