Abstract

Objective. To study the effectiveness of restoring respiratory parameters when using vibration PEP therapy in patients with chronic obstructive pulmonary disease (COPD) after coronary artery bypass grafting (CABG).

Material and methods. 58 patients who underwent CABG were examined. Patients were divided into 4 groups: I – normal respiratory parameters, II – after CABG with initially normal parameters, oPEP therapy was performed, III – patients with COPD, IV – after CABG surgery, oPEP therapy was performed in patients with COPD. All patients underwent spirometry using SMP-2101-R-D apparatus (Russia). Statistical analysis of the data was carried out using the Statistica 10 software.

Results. On the second postoperative day, patients in all the studied groups showed a marked decrease in the volumevelocity parameters of the respiratory flow, compared with preoperative studies. While on the fifth day, the volume-velocity parameters in patients of group IV improve, and in patients of groups I–III they remain reduced. Thus, in the fourth group, the vital capacity reaches the preoperative level, whereas this parameter in the Group I remains reduced by 28%, in the Group II – by 28%, and in the Group III – by 40%. On the ninth postoperative day, vital capacity and functional vital in groups II and IV reach the preoperative level, whereas in groups I and III these parameters remain reduced by 30%, 32% and 36%, 38%, respectively.

Conclusion. Conducting vibration PEP therapy after CABG surgery in patients with COPD allows to restore volumetric respiratory parameters by the time of discharge from the hospital, whereas without load spirometry, recovery does not occur.
