Abstract

An aortic dissection is one of the most dangerous diseases in cardiovascular surgery. Conservative treatment usually is used as therapy of stable patients with aortic dissection without complications. In 30% of cases, this disease develops a syndrome of malperfusion visceral organs and kidneys, resulting in possible decompensation of the functions of these organs. Surgical treatment for aortic dissection are shown in complicated: in the case of aortic rupture, formation of aneurysms of the thoracic aorta, the development of the syndrome of malperfusion digestive organs, kidneys, spinal cord and lower extremities. Currently, there are many methods of treatment of aortic dissection and various surgical tactics in relation to this disease. Open surgical fenestration is one of the methods of surgical treatment of patients with aortic dissection complicated by development of syndrome of malperfusion of the abdominal cavity, kidneys or lower extremities. The concept of the method is based on creating a single lumen of the aorta by excision of the intimal delaminated in a certain segment of the aorta.
