Abstract

Objective. Search for early ultrasound markers of acute kidney injury (AKI) and study of their relationship with echocardiography data in patients in the early postoperative period after coronary artery bypass grafting surgery (CABG).

Material and methods. The analysis included 108 patients before and after CABG: 14 (13%) with AKI and 94 (87%) without complications. All patients underwent ultrasound examination of the kidneys with Doppler scanning and echocardiography before and on the first day after surgery.

Results. The analysis revealed a significant decrease in speed values, as well as an increase in the resistive index (RI) and pulsatility index (PI) in the group with AKI, compared with the control group (p < 0.001). In the AKI group (n =14) a direct correlation between RI and creatinine level was found (R = 0.830, p < 0.05). ROC-curve analysis for the RI threshold for the diagnosis of AKI showed 0.76 with sensitivity 71% and specificity 96% (AUC = 0.930). Comparative assessment of renal Doppler data and echocardiography in the AKI group revealed an inverse correlation between RI and cardiac index (R = -0.874, p < 0.05).

Conclusion. The use of comprehensive ultrasound for heart and kidney hemodynamic assessment allows the early diagnostics of AKI as well as dynamic monitoring of patients in the early postoperative period after CABG.
