Abstract

Acute kidney injury (AKI) is heterogeneous and multifactorial clinical situation with wide variability of syndromes. Understanding the etiology and pathophysiological mechanisms of the development of renal dysfunction, taking into account anamnestic data, analysis of comorbid causes and risk factors, provides the opportunity for a personalized assessment of AKI to determine the protocol for managing each individual patient. We have considered various categories – prerenal, renal and postrenal AKI, in particular, touched upon the main aspects of the development of acute organ damage in cardiac surgery patients at different stages of treatment. In the cohort of severe patients with the development of AKI, a high degree of disability and mortality is expected. With regard to patients operated on the heart and blood vessels, the development of AKI worsens the results of cardiac surgery, and among patients requiring replacement of kidney function, the mortality rate can reach 70%.
AKI is a serious clinical situation that can be attributed to an interdisciplinary problem, and therefore teamwork of specialists of various profiles (cardiac surgeons, cardiologists, nephrologists, internists, endovascular surgeons, etc.) is necessary.
