Abstract

In this review article, all the main stages of the chronic thromboembolic pulmonary hypertension (CTEPH) development, modern diagnostic methods, and treatment are covered. The importance of evaluating the feasibility of performing pulmonary thromboendarterectomy (PTE) in an expert center is emphasized, which is a radical treatment method involving the removal of post-thrombotic material from the branches of the pulmonary artery. In cases where surgery is not feasible for patients with inoperable CTEPH, a transcatheter treatment method is proposed in the form of sequential balloon pulmonary angioplasty and specific medical therapy. The article provides a detailed description of modern criteria for selecting patients, defining indications and contraindications for PTE. Special attention is given in the publication to the possible effects of the duration of CTEPH history on indicators of pulmonary circulation hemodynamics, hospital mortality rate, and frequency of perioperative complications.
