Abstract

Pharmacological inhibition of proprotein convertase subtilisn/kexin type 9 (PCSK9) is a state-of-the-art mechanism to reduce low-density lipoprotein cholesterol and the risk of cardiovascular complications. The systematic review analyzing outcomes from randomized controlled trials and post hoc data published in 2023−2025 on PCSK9 inhibition therapies is presented. The purpose of the systematic review is to assess the current data on the safety and efficacy of used PCSK9 inhibitors, as well as to highlight trends in the development of new strategies for PCSK9 function inhibiting. The systematic search was conducted in the PubMed database (combining data from the MEDLINE, PreMEDLINE, NLM systems) using the descriptor “PCSK9” in the search order, according to the thesaurus of the MeSH (Medical Subject Headings), as well as the corresponding time interval and the randomized controlled trial criterion. As a result of the search, 38 publications were found. The methodological quality of the studies was assessed according to the criteria of the Cochrane Community. 38 trials are included in the analysis. The 2023−2025 studies confirmed that long- term inhibition of PCSK9 by monoclonal antibodies, small interfering RNA inclisiran treatment are safe and provide a reduction in cardiovascular risks. At the same time, new strategies that are being developed can expand the therapeutic possibilities in this area.
