Abstract

Relevance. Acute limb ischemia (ALI) results from an acute (<2 weeks), sharp decrease in arterial perfusion to a limb and can lead to limb loss and death. This condition is urgent and requires rapid revascularization using endovascular, surgical, and hybrid treatment methods.

Objectives. The aim of this study is to improve treatment outcomes for patients with acute lower limb ischemia. Material and methods. To address this objective, surgical treatment outcomes were analyzed in 184 patients with acute lower limb ischemia from 2016 to 2024. Patients were divided into two groups based on the type of revasculariza-tion procedure performed: group I – hybrid interventions; group II – isolated open interventions.

Results. Statistically significant differences were found in the incidence of thrombosis in the postoperative period during the 30-day follow-up. Hybrid procedures significantly reduce the risk of developing arterial thrombosis in the postoperative period (p = 0.004). The rate of limb amputation within 30 days was also lower in the hybrid group: 11 cases (9.7%) versus 12 cases (16.9%) in the open group. However, this difference was not statistically significant (p = 0.23), meaning the limb salvage rate was comparable in the two groups: 90.3% versus 83.1% in groups I and II, respectively.

Conclusion. The data from this study demonstrate that the results of hybrid surgical treatment for patients (group I) with acute lower extremity ischemia are significantly better than those of open procedures (group II).
