Abstract

Evidence of heart pathologic deformity associated with preoperative mitral valve (MV) defects has direct influence on intensity and restoration degree of heart geometry at the late period after surgical correction of valvular pathology. Material and methods. Assessment and analysis results of heart cavities remodeling in 368 patients with mitral valve acquired pathology depending on its character and intensity at the close period and within 5 years following replacement of mitral valve. Follow-up of the patients was carried out using ultrasonographs HDI, Acuson and Sonos-2500 («Hewlett Packard») with transthoracic sensors 2.5 and 3.5 MHz with one- and two-dimensional regimes. The study used remodeling types classification depending on basic value of volume remodeling index (VRI) with retrospective analysis of the late surgical results. Results. Statistical and prognostic relationship between VRI value and character and severity of the basic pathology of mitral valve (p<0.05) as well as significant influence of hemodynamic and structural geometric characteristics of the basic mitral valve pathology on the intensity and completeness of heart geometry restoration at the close and late follow-up periods were revealed. Conclusion. So the less the basic heart geometry and intracardiac hemodynamics are compromised the more quick and complete heart geometry is restored following mitral valve replacement.
