Abstract

Objective of the work - evaluation of correlation between cardiac hemodynamics and state of the structural functional levels of the cerebral blood flow associated with hypertensive encephalopathy (HE). Material and methods. Transthoracal echocardiography and ultrasonography of the cerebral vascular system based on the concept of its plotting on five structural functional levels were carried out in 209 patients with HE of I-III stage. Results. It was found that structural changes of cardiac muscle and diastolic cardiac function impairment aggravate while systolic function remains intact as HE aggravates, various changes at all structural functional levels of the cerebral vascular system are formed. Vascular structural changes, blood flow depression, peripheral resistance increase, autoregulation mechanisms impairment, venous discirculation were revealed; interaction between structural and functional cardiac values and hemodynamics values on all structural functional levels of the cerebral vascular system was revealed. Conclusion. Cardiac and vascular cerebral pathologic processes associated with HE develop and progress simultaneously; besides, they depend upon one another. Complex evaluation of cardiac hemodynamics and state of cerebral blood flow structural functional levels considered as unified system based on the clinical and instrumental techniques, will serve to reveal the logic of the genesis and progress of the disease, which makes it possible to make objective choice of therapeutic approach for HE patients.
