Abstract

Objective of the work - detection of objective blood flow changes of the cerebral venous system in patients with hypertensive encephalopathy (HE) and determination of reliable ultrasound criteria of venous discirculation. Material and methods. Detailed clinical and instrumental study of 65 patients with hypertensive encephalopathy was carried out. 35 out of them had constitutional venous insufficiency and 30 patients had not; 30 healthy persons made up control group. Blood flow of internal jugular veins, direct sinus and superior eye veins was studied with contemporary ultrasonic devices in combination with Valsalva test and orthostatic sign. Results. Results of Valsalva test and orthostatic sign showed that retrograde blood flow in all superior eye veins was revealed in all the 35 patients of the group with constitutional venous insufficiency. Ostial valves dysfunction of internal jugular veins was revealed practically in all the patients of the same group with «constitutional phlebopathy» (CPP) (as opposed to another group of patients without it and control group). Conclusion. Accordingly there were revealed objective signs of a new type of HE, that is venous hypertensive encephalopathy, diagnosis of which should affect the choice of therapeutic approach for HE patients.
