Abstract

In spite of upgrading of devices and oxygenators for CPB it was certainly impossible to solve all the problems and avoid complications associated with its performance. One of the important problems is investigation of CPB techniques, in particular the influence of low-intensity laser radiation (LILR) on blood cells, tissues and organs metabolism during cardiopulmonary bypass. The objective of the investigation was to evaluate the influence of low-intensity laser radiation on the morphology of red cells function. Material and methods. Nonrandomized prospective clinical study included 47 patients with congenital heart defects (CHD). Blood low-intensity laser radiation with wave length 405 nm and power 1.5 mWt was carried out in 17 patients for 7 minutes during perfusion. The state of hemodynamics, metabolism (lactate, blood glucose), acid-base balance (ABB), diuresis and erythrocytolysis during CPB were considered when evaluating ВЛОК-405 potency. Results. Application of LILR with wave length 405 nm was accompanied by improvement of erythrocyte state, mean hemolysis reduction fr om 2.9 ± 0.7 to 1.9 ± 0.9 g/l. CPB organs and tissues metabolism was also improved which was proved by lower lactate rates - mean 2.4 ± 0.2 mmol/l with 360 min. CPB versus the group without LILR wh ere mean lactate rate was equal to 3.1 ± 0.3 mmol/l. Conclusion. Application of LILR with wave length 405 nm, power 1.5 mWt, radiation time 7 min during CAPB cardiosurgical procedures in CHD patients improves persistence of erythrocyte membrane to the impact of rolling pumps of PO and extracorporeal contour. Metabolism of organs and tissues due to adequate oxygen transfer was owing to this effect. Positive effect of LILR on blood corpuscles makes it possible to perform more long-term perfusions in patients with severe CHD with more adjusted to physiology approach and with minor complications during CAPB and postoperative period.
