Abstract

Objective. The aim of the study was to evaluate endothelial function in patients with hypertensive encephalopathy (HE) during antihypertensive treatment (AHT). Material and methods. 107 patients with hypertensive encephalopathy of grade I - III, aged from 37 to 75 (mean age 52 ± 10.7 years) were observed before treatment and in dynamics, in 6 months after initiation of antihypertensive therapy. Control group included 30 patients. 24-hour monitoring of arterial blood pressure with efficacy endpoint for antihypertensive therapy were performed. Endothelial function was assessed using ultrasound cuff test. Results. Disturbance of endothelial function was observed at all stages of HE without improvement during AHT. According to 24-hour monitoring of arterial blood pressure (MABP) during 6 months-period, time-averaged rates of arterial pressure were consistent with control values; the increasing number of patients with normal degree of nocturnal reduction of arterial pressure was noted; rate of velocity for pressure rise in the morning was consistent with control values in patients with hypertonic encephalopathy of I and II grade; the increasing number of eutonic patients with vegetation Kerdo index of stage I was noted. Negative correlations between endothelial function and arterial pressure values characterizing pressure load and vegetative status of patient, were noted. Conclusion. AHT planning and control of its efficiency in HE patients must be carried out on the basis of endothelial function state under 24-hour MABP guidance with correction of ABP values leading to endothelial damage (pressure load, vegetation Kerdo index).
