Objective. To evaluate the prognostic significance of the indices of longitudinal deformation of the myocardium in patients with coronary artery disease with severe ischemic myocardial dysfunction of the left ventricle based on the analysis of their dynamics before and after revascularization.

Material and methods. 45 male patients with multivessel disease were enrolled in study. All patients underwent standard echocardiography and speckle tracking echocardiography to evaluate the longitudinal left ventricular (LV) myocardial deformation, as well as to assess the symptoms of chronic heart failure (CHF), according to the NYHA classification before and 6 months after the CABG surgery.

Results. Six months after the surgery all patients had correlations of the baseline indices of the longitudinal LV myocardial deformation and the trends of CHF symptoms after the syrgery. If the baseline global longitudinal deformation was less than –6.3%, there was a lack of significant positive trends in CHF symptoms in the postoperative period.

Conclusions. Longitudinal systolic left ventricular myocardial deformation is a reliable quantitative measure of the LV systolic function. The speckle-tracking echocardiography can be used to determine if surgical revascularization is suitable in patients with coronary heart disease and low left LV ejection fraction.
