Stylokarotid syndrome as a cause of acute occlusion and aneurysm of the internal carotid artery

Abstract

The presented clinical case describes the tactics of management and surgical treatment of a patient with an internal carotid artery (ICA) aneurysm that developed as a result of dissection of the internal carotid artery, the cause of which was an elongated styloid process.

In the acute period of development of dissection and occlusion of the ICA, conservative treatment was carried out. The patient was stabilized, and an ICA aneurysm formed. After 76 days, an operation was performed: aneurysmectomy of the right ICA with end-to-end anastomosis. Resection of the styloid process on the right.

Considering that restoration of blood flow during acute occlusion of the ICA can lead to hemorrhagic transformation of the ischemic focus and death of the patient, conservative treatment is advisable in the acute period of stroke.

Delayed re-evaluation of vessel patency and further open surgical treatment (aneurysmectomy) can completely restore the patency of the affected artery, eliminate the cause of ICA trauma (resection of the styloid process) and prevent possible recurrent ischemic events.
