Abstract

The aim of study was to improve Doppler evaluation of penile circulation. We observed 88 patients with erectile dysfunction and 30 healthy volunteers (control group). Accessory penile arteries were found in 26,3% investigated males. Rates of changes in the diameter and flow velocity in accessory and common penile arteries after onset of pharmacological erection were comparable. Unlike the control group 37 patients with venogenic erectile dysfunction dont experience decrease in diameter and flow velocity of preprostatic venous plexus after onset of pharmacological erection. Transrectal Doppler and dynamic cavernosography with cavernosotonometry (gold standard) data analysis of shunt in dorsal penile vein and preprostatic venous plexus (using compression tube) showed satisfactory sensitivity and specificity of this Doppler ultrasonography method (82,6 and 88,8% respectively). Transrectal pharmacoechodopplerography of accessory penile arteries and preprostatic venous plexus is highly informative, noninvasive and comparatively simple means for assessment of penile blood flow.
