Abstract

Despite the significant achievements in modern cardiac anesthesiology and cardiosurgery, a high risk of cerebral complications in open heart surgeries under cardiopulmonary bypass is presented. A number of patients after adequate revascularization in early postoperative period has a worsen state, connected with reperfusion syndrome development that depends on the initial regional microcirculation. Postoperative neurologic disorders occur as focal neurologic disorders and as negative dynamics of cognitive processes. The aim of this study was to find out the changes of tissue perfusion in the brain that are at the basis of reperfusion syndrome in patients after coronary artery bypass graft (CABG) under CPB and correction with dynamics of neurologic state. Neurologic examination and perfusion CT were conducted during 3-5 days before CABG, on the second day, 8th and 10th days after coronary bypass surgery with CBF, CVT, TTP. Significant improvement of values after perfusion CT is mostly shown in border areas with related blood flow (in areas of blood flow linked to risky). Significant correlation of CBV change in Wernicke`s area with a number of neurologic syndromes before CABG shows their relationship and possible influence of the initial neurologic state on the state of patients, so possibly, it must be taken into account while planning the procedure under CPB.
