Аннотация

Аневризма брюшной аорты (АБА) отмечается среди мужского населения в возрасте 45–54 лет в 1,3–1,5% случаев, в возрасте старше 75–80 лет частота встречаемости увеличивается в 10–12 раз и составляет 12,5–14%. У женщин распространенность АБА – от 0 до 5,2–6%. Техника «дымохода» при эндоваскулярном лечении АБА (chimney technique in endovascular repair of abdominal aortic aneurysms, ChEVAR) была впервые использована при экстренных и неотложных состояниях для лечения разрывов АБА, а в настоящее время успешно применяется при плановых вмешательствах.

В статье рассматриваются текущие литературные данные о ChEVAR из различных клиник мира. Основное внимание уделяется клиническим результатам, техническим примечаниям и сравнениям с другими методами. НовыеданныеоChEVARпоказываютблагоприятныерезультатылечениясубренальных(юкстаренальных)иинтерренальных (параренальных) АБА. Адекватный размер эндопротеза, связанный с длиной шейки аневризмы, имеет основополагающее значение для предотвращения эндолика Iа типа. Методику «дымохода» можно успешно использовать для лечения субренальных и интерренальных аневризм аорты при определенных анатомических условиях. Однако развитие эндолика Iа типа после процедуры представляет существенную проблему, поэтому необходимы дальнейшие исследования для определения безопасности и эффективности метода.
