Abstract

The aim of the work was to evaluate the diagnostic accuracy of ultrasound and determine the causes of errors in determining the degree of ICA stenosis.

Materials and methods. A single-center retrospective comparative study was conducted. The study included 602 patients (1179 arteries) with a full examination (ultrasound + angiography).

Results. Based on the analysis of the results of ultrasound and angiography of the carotid arteries, a comparative assessment of the diagnostic accuracy of hemodynamic and planimetric determination of the degree of ICA stenosis was carried out, modified hemodynamic criteria for ICA stenosis with greater diagnostic accuracy were identified, the causes of errors in ultrasound were determined, and factors influencing the hemodynamic parameters of blood flow were determined.

Conclusion. Our study demonstrated high sensitivity and specificity of the use of complex planimetric assessment and assessment of hemodynamic parameters in the diagnosis of ICA stenosis. However, further research is needed to standardize the hemodynamic criteria for varying degrees of ICA stenosis. We have systematized the causes of ultrasound errors. It is necessary to take into account the factors potentially influencing the ultrasound results, especially at threshold values of the degree of ICA stenosis, when errors can lead to the choice of the wrong treatment tactics.
