Abstract

Тhe natural course of the subarterial ventricular septal defects (VSD) is frequently complicated by the aortic valve (AV) insufficiency, and this complication represent 2 to 12% of all VSDs expanding in the Asian countries mostly (up to 30%). The purpose of the present study was to investigate morphological features of the interventricular septum and the aortic valve in VSD associated with the AV insufficiecy compared to normal and rheumatic specimens. The specimens of the aortic valve and the interventricular septum obtained by gross microscopy with semislim epoxide slices and scanning electron microscopy allowed to reveal some similarities and differences in these diseases. For instance, rheumatic changes of the interventricular septum were more inflammatory, and infiltration was associated with moderate swelling, whereas the interventricular septum in VSD and the AV insufficiency was more oedematic, with myocytes vacuolization. The more significant changes were discovered in the AV structure: rheumatic AVs were characterized by desquamation of the endothelial cells and by many different deposits of calcium salts, whereas incompetent AVs in VSD presented exfoliation of the fiber fascicles and structure deficient zones that led to stretching and thinning of the leaflets and their perforations.
