Abstract

Intensity of interstitial myocardial fibrosis in patients with dilated cardiomyopathy (DCMP) is liable to affect integrity of metabolic processes in the myocardium of the left ventricle (LV) and can be adequately evaluated with 18F-ФДГ positron emission tomography of myocardium. The main task of this investigation was to evaluate prevalence of perfusion and metabolic disorders in this category of patients. Diagnosis of myocardial viability was conducted using single-photon emission computed tomography (SPECT) with 99mTc-tekhnetril and positron emission tomography (PET) with 18F-ФДГ. 50 examined patients with DCMP were divided into groups depending on the localization of perfusion (PD) and metabolic defects (MD). It was revealed that local cardiac fibrosis pattern most probably resulted from previous myocarditis had been observed in the patients with predominant local defect of isolated ventricular walls and low metabolic heterogeneity. Fibrous changes diffusion most probably was observed in patients with high metabolic heterogeneity, perfusion-metabolic disorders of anterioposterior localization and diffuse metabolic decrease. It was revealed that perfusion-metabolic indices were characterized by more marked deviations and associated with more severe LV dysfunctions associated with disorders of anterioposterior localization.
