Abstract

Reciprocal interaction of the left and right ventricles is determined by the term interventricular relations. Anatomical unity of both ventricles, associated with their total anatomical structure is at the heart of this phenomenon. Contractility restoration (improvement) of one of them with inadequate function of the other can result in its preload growth which the latter may not manage with. This results in its failure. For biventricular heart failure it is essential to establish the weakest link and only after that make arrangements to increase efficiency of the most damaged ventricle being on the alert for preload and contractility of the other ventricle. First signs of decompensation of the less damaged ventricle (enddiastolic volume increase above the initial, decrease of stroke volume and ejection fraction) must result in the measures aimed at the change of the therapeutic approach. It is essential to decrease preload for the weakest ventricle and increase its contractility.
