Abstract

Objective. To estimate the efficacy of application of calcium sensitizer in the intensive care for cardiac surgery patients with the low cardiac output syndrome caused by the biventricular insufficiency.

Material and methods. 80 patients of 18 to 76 years old were included in the prospective randomized monitored research. Within the years 2008 to 2011 in the condition of the cardiopulmonary bypass and pharmaco-hypothermal cardioplegia the patients underwent the cardiac surgery interventions complicated by the low cardiac output syndrome in the post-operative period. 41 patients included in the main group additionally to the traditional treatment were cured by synchronous or repeated 24 hours infusion of levosimendan. 39 patients included in the control group were cured by traditional therapy for cardiac failure. In the present research the medicine was infused within the first hours of patient's admission into the intensive care unit without the loading dosage. 12.5 mg of levosimendan were infused into each patient.

Results. Six hours later after the infusion of the medicine at the dosage of 0.1 mcg/kg/min the trustworthy changes of echocardiography records were reported as well as the central and systemic hemodynamics with simultaneous decreasing of tension ofcardiotonic therapy. The number of lethal outcomes has been decreased.

Conclusion. The results of the study allowed to conclude that calcium sensitizer levosimendan should be used in complex treatment of patients with the low cardiac output caused by biventricular failure after cardiac surgeries and can be well combined with all medicines.
