Comparative evaluation of the diagnostic efficiency of different radiological methods of the examination of brachiocephalic arteries

Abstract

Objective. The purpose of this study is a comparative evaluation of the diagnostic efficacy of different radial methods of examination critical stenosis of the carotid arteries and vertebrobazillary pools: digital subtraction angiography (DSA), computed tomographic angiography (CTA) and magnetic resonance angiography (MRA).

Material and methods. In A.N. Bakoulev Scientific Center for Cardiovascular Surgery of the Russian Academy of Medical Sciences was held 441 (100%) of the angiographic studies which DSA 391 (88,7%), CTA and unenhanced MRA – 50 (11.3%) research. Comparative characteristics of the diagnostic value of DSA, CTA and unenhanced MRA analyzed using methods developed for testing interpreting in medical settings by means of a triple scheme qualifications, in determining the degree of critical stenoses 75–99% for the internal carotid and vertebral arteries.

Results. On the basis of the generalized parameters of the diagnostic efficiency of all methods used angiography is the decrease of the index of sensitivity to vertebrobazillary pool relatively CP (carotid pool) 10.1%, specificity index by 6.1% and index positive statements by 5.5%. When comparing the general indicators of the diagnostic value of DSA, CTA and unenhaced MRA, it is established that the generalized index of sensitivity, specificity and positive statements was 97.6, 93.7 and 84.7% respectively.

Conclusion. Ultrasound Duplex Scanning with DSA remains the most sensitive and informative method of visualization occlusive diseases brachiocephalic artery (BCA). CT angiography is recommended for the study of critical stenosis and occlusions of the BCA for clarification of localization highly located atherosclerotic plaques. Unenhanced magnetic resonance imaging angiography should be used if you are allergic to contrast agents.
